How do I apply?

Before starting your application, please read ‘what we look for in an ethics application [PDF 69KB]’.

1. Complete the QM Ethics application. We have a guide on how to avoid common mistakes [PDF 341KB].
2. Upload the relevant documentation which may include a QMERC Participant Information Sheet template [DOC 58KB] and QMERC consent form template [DOC 48KB].
3. Your application will be automatically submitted only when all signatures are in place.
4. Upon receipt of your application, the team will do a completeness check. Incomplete applications will be sent back.
