How do I apply?

Before starting your application, please read our Research ethics checklist - required information and considerations [PDF 76KB] 

1. Complete the QM Ethics application. We have a guide on how to avoid common mistakes [PDF 330KB].
2. Upload the relevant documentation which may include a Participant Information Sheet template [DOC 66KB] and Consent Form template [DOC 56KB].
3. Your application will be automatically submitted only when all signatures are in place.
4. Upon receipt of your application, the team will do a completeness check. Incomplete applications will be sent back.
